How to Send a Referral
What to Include
To help us provide the best care for your patient, please include relevant clinical details with your referral.
- ✓Current medications
- ✓Treating team members (GPs, surgeons, trainers)
- ✓Specific service requests (e.g., orthotic assessment, return-to-sport planning)
- ✓Relevant clinical history and presenting complaint
- ✓Any imaging or investigation results
Funding-Specific Referral Requirements
Medicare
Requires one of the following:
- •GP Management Plan
- •Team Care Arrangement
- •CDM Referral Form with allocated visit details
DVA
Requires a written referral on GP letterhead including:
- •Client name, DOB, address, and contact details
- •DVA number
- •Service type and reason for referral
WorkSafe
No referral is necessary, however a basic referral with client and injury details is preferred to help us provide the best possible care from the first appointment.
TAC
Referral is recommended prior to therapy starting. Should include:
- •Client particulars and reason for referral
- •Relevant history and specific requests
- •TAC claim number
